Simply click on the "forgot my password" link on the sign in box. A new box will then appear, which will prompt you to enter your Patient Portal sign-in email. Once completed, an email will be sent with a password reset link.

With Releaf+, you don't have to worry about being locked into your subscription, you can cancel anytime. To cancel your subscription, log in to your Releaf account. Once logged in, click the “Subscriptions” tab. This tab is on the left-hand side of the screen, below the “Home” tab and above the “Online Shop” tab. Then click the “Cancel Service Subscription” button, which you will find at the bottom right-hand corner of the page

Your SCR is your medical records. Summary Care Records (SCRs) have two primary forms: the core SCR and the SCR with Additional Information (SCRai). The core SCR contains essential patient information, including full name, date of birth, NHS number, long-term health conditions, significant medical history, current medications, allergies, and any known adverse reactions. Approximately 58 million people have an SCRai, which includes additional information. SCRais document lifestyle details like smoking status, alcohol consumption, treatment preferences, and necessary adjustments during treatment. They also explain the reasons behind prescribed medications and include information about hospitalisations, immunisations, and relevant test results to ensure appropriate patient care.

All our consultations are conducted by video call in the comfort and privacy of your own home. This means you can be seen by a doctor who specialises in your condition, no matter your location and at a time that is convenient for you. Once you have booked your consultation, you will receive an email confirming the date, time and a link to join on the day. To prepare for appointment you should find a quiet place where you can talk openly about your health and bring along the ID you used to book your appointment, so the doctor can confirm your identity Our doctors are compassionate and knowledgeable, they will take the time to listen to your needs and, if you are eligible, find a treatment plan that suits your condition and lifestyle. Signing up with Releaf is easy. First, head over to our medical cannabis eligibility checker. This takes less than a minute to complete. If you are deemed to be potentially eligible for a medical cannabis prescription, you will then need to: Create a Releaf accountAnswer all the questions in the health questionnaireSupply your medical records (we can reach out to your GP on your behalf, or you can supply the records yourself)Complete the ID check. Once this is complete, your information will be assessed by our triage team and you will be prompted to book your initial consultation

During your consultation, your clinician will suggest a suitable treatment plan based on your condition and lifestyle. Once the doctor has written your prescription, you will be notified and then able to order your medication within your patient portal. If you wish to reduce your medication quantities, you can do this before you check out. Your doctor will be notified of the change so we can continue to provide you with the best care.

All Releaf patients receive follow-up consultations every 3 months, to make sure that their prescribed medical cannabis options are working effectively. If you need any support outside of these follow-up consultations, feel free to reach out to our Patient Support Team at support@releaf.co.uk - Our office hours are 9 AM to 5 PM, Monday to Friday.

You can reschedule your appointment via your Patient Portal by clicking the ‘Appointments’ tab on the left-hand side. You can then choose to edit your current appointment time. If you need help with rescheduling your appointment, contact the Patient Support Team at support@releaf.co.uk

All prescription items will be sent out with a photocopy of the original paper prescription. You can also access all previous prescriptions by logging into your Patient Portal.

If we find that your Summary of Care Record does not contain all the evidence we need to ensure that a medical cannabis prescription is the right course of treatment for you, our clinicians may ask for supporting evidence. This can include evidence of medications you have taken in the past, treatments you have undergone, or evidence of the diagnosis that you are seeking treatment for.
